An applicant whose primary reason for traveling to the United States is for medical treatment or evaluation should submit the documents described in the "Basic documents and photographs" and provide the appropriate consular section with the following information for adjudication of his/her visa:
- A medical report statement from a physician stating the nature of the medical condition and the possibility of treatment in Poland;
- A statement from a licensed American physician, on official letterhead or hospital letterhead, describing:
- the medical problem based on documents sent from Poland;
- the treatment required;
- the cost of the treatment, in the worst-case scenario;
- the estimated time the treatment will require, in the worst-case scenario;
- the name of the person the physician believes will pay the cost of therapy or treatment;
- the relationship between the physician and the institution where the treatment will take place; and
- how much of the treatment is in-patient and how much is outpatient.
If relatives in the United States are paying, bank statements for the last six months from the U.S. sponsor are required. Bank letters are not acceptable. If the applicant is paying, the applicant should provide evidence of financial resources in Poland.
Applicants should not send these documents to the Embassy or Consulate General directly as we do not have the storage space for them, Instead, applicants should bring this information with them at the time of their visa interview. However, the letter from American physician on official letterhead must be sent directly to the Non-Immigrant Visa Section of the Embassy in Warsaw at fax number 011-48-22-504-2532, or to the Visa Section of the Consulate General in Krakow at fax number 011-48-12-424-5103. Before the visit to the Consulate please confirm the receipt of the fax by phone (011-48-22-504-2000 in Warsaw and 011 48-12-424-51-00 in Krakow). Even if complete information regarding the medical condition and proposed treatment is presented, it does not guarantee the issuance of a visa. All visa applicants, even those going to the United States for medical treatment, must prove that they are not intending immigrants.
