Applications for Children

Children under the age of 14 do not have to appear in person for a visa interview. If one of their parents has a valid American visa, the child’s application and all Required Documents can be delivered to the Information Window of the Embassy Monday through Friday 8:30am to 12:00noon. The documents can also be sent to the Embassy by DHL Courier Service. A copy of a parent’s visa must be included with the application. Parents must not forget to sign a child's DS-156 form. For VWP countries a copy of a parent's Machine Readable Passport (MRP) is required. A MRP issued on/after Oct. 26, 2005 must have a digital photo.

The U.S. Embassy in Warsaw reserves the right to request a personal interview of parents.

In cases where neither parent has a visa, a parent will have to apply in person on behalf of the child. He/she can make the appointment by following the procedures outlined in the How to Apply section of this site.

Minors over the age of 14 are required to apply for visas in person. We suggest that one parent accompany minor appllicants to the interview, although the presence of a parent is not strictly required.
